Dynamique de diffusion/adoption de l’innovation en imagerie multimodale et interdépendance des communautés médicales: situation de la TEP-TDM en oncologie

Résumé

With more than half a million examinations per year, PET-CT examinations have been the most common type of examination performed in French nuclear medicine departments for several years. In a decade, the number of PET cameras has doubled and the number of PET examinations has tripled in France. However, the number of PET examinations remains 10 times lower than the number of scans performed, this factor being comparable to the number of nuclear physicians compared to the number of radiologists. While the medico-economic data concerning the benefits of integrating PET-CT into the management of oncology patients are accumulating, this scientific reality contrasts with the observation made in the field of unequal adoption of multimodality imaging procedures by the medical community. 20 years after the emergence of the first hybrid PET-CT cameras, in the era of digital PET, the factors that have facilitated or slowed down the organisational changes inherent in the introduction of these innovations remain at the level of hypotheses, but have not been the subject of in-depth scientific work. The regional analysis of the dynamics of PET-CT in mainland France is part of a research project on the dynamic of medical innovation and the professional changes in nuclear oncology. With the emergence of new radiopharmaceuticals and theranostics, it has become essential to understand the socio-organisational determinants facilitating the adoption and diffusion of innovative solutions proposed by the nuclear medicine community.
