AIDS and health-policy responses in European welfare state

The article analyses the complex policy issues set into motion by the AIDS epidemic in Europe. It shows how different health systems responded to the challenge and how the management of an epidemic influenced general trends in health care policies. AIDS emerged as an ill-structured problem calling into question the central position of the medical profession in policy making, the institutional fragmentation in the health sector and the exclusive role of economic factors in the reform policies. Whilst the mainstream health services had to adapt to new demands, public health services had to reinvent themselves to cope with the specific issues around prevention. The analysis thus distinguishes between the well-established medical care sector and the weakly structured public health sector. It focuses on the reshaping of policies in this previously neglected field, including at the level of the European Union.
Analyse des enjeux politiques complexes que l'épidémie de SIDA a déclenchés en Europe. On montre comment différents systèmes de santé ont répondu à ce défi et comment la gestion d'une épidémie a influencé les tendances générales dans les politiques de soins de santé. Le sida a émergé comme un problème mal structuré mettant en question la position centrale de la profession médicale, la fragmentation institutionnelle du secteur de la santé et le rôle exclusif des facteurs économiques dans les politiques de réforme. Alors que les principaux services de santé devraient s'adapter à de nouvelles demandes, les services de santé publique devrainet eux se réinventer pour faire face aux enjeux spécifiques concernant la prévention. L'analyse distingue donc entre le secteur de soins médicaux bien établis et le secteur de soins de santé public faiblement structuré. Il se centre sur les reformulations des politiques dans ce dernier secteur qui a souvent été négligé y compris au niveau de l'Union européenne.
